CMH Service Provider Guide

How to Use this Guide to Promote Consumer CHOICE

Just as each consumer has individuals needs, each provider in our network has special qualities. When more than one provider is available for the service authorized for you, you may choose the organization that will best meet your unique needs.

Definitions of mental health services, and the ‘group' they are available to, are displayed on the left side of the page. The agencies or individuals we contract with to provide the service are listed on the right side of the page.

Vocational Skill Building (DD, MI)
This program will assist you to prepare for and achieve economic independence. It includes work site activities and employment services related to your Person- Centered Plan.
You can identify the providers available in your network when you see your county's logo beside the provider's name:

MI, DD, and C refer to who may use the service described.
MI means services to individuals with Mental Illness,
DD means services to individuals with Developmental Disabilities, and
C refers to Children's Services (17 yrs and younger).
