Medical Resources Dental Resources

City on a Hill Ministries Health Clinic – 100 S. Pine St, Zeeland (616) 748-6009 located in the old Zeeland Hospital. Free medical attention for minor injuries & illnesses; must have ID and no health insurance. Walk-in clinic every Tuesday with registration from 5:30 to 6:00.

City on a Hill Women’s Health Initiative - 100 S. Pine St, Zeeland (616) 666-0215 by appointment ONLY. Free mammograms, pap tests & health exams for women in post-childbearing years; call for eligibility and information. Services scheduled for 3rd Thursday of each month.

Cherry St. Health Center - 550 Cherry St, Grand Rapids (616)235-7272. Multiple locations in the Grand Rapids area. Uninsured fees based on ability to pay; Medicaid provider.

InterCare Clinic – 285 James St, Holland (616) 399-0200. Medicaid provider; uninsured fees based on ability to pay - $20 minimum; alternatives for those who may not be able to afford fee.

Holland Community Health Center – 336 South River, Holland (616) 394-3788. Medicaid provider; uninsured fees based on ability to pay - $15 minimum; alternatives for those who may not be able to afford fee.

Ottawa County Health Department – Three locations: Holland - 12251 James (616)396-5266; Hudsonville - 3100 Port Sheldon (616) 669-0040; Grand Haven - 16920 Ferris (616) 846-8360. Children’s Special Health Care prog., immunizations, vision and hearing screenings for children birth to pre-school, family planning services/contraception, sexually transmitted disease testing, and Maternal-Infant Health program. Medicaid accepted; other fees based on ability to pay.

Muskegon Family Care - Two locations: 2201 S. Getty St, Muskegon (231) 739-9315 and 1700 Oak Ave, Suite 400, Muskegon (231) 777-2093. Medicaid provider; uninsured fees based on ability to pay.

Hackley Community Care - 2700 Baker St, 3rd floor, Muskegon Heights. (231) 737-1335. Medicaid provider; uninsured fees based on ability to pay.

Holland Free Health Clinic – 99 W 26th St, off Michigan Ave. (by Holland Hospital) (616) 392-3610. Free exam and dental cleaning; limited dental referrals for additional services.

InterCare Clinic – 285 James St, Holland (616) 399-0200. General dental services provided based on ability to pay, $25.00 minimum (alternatives for those who may not have the $25.00). Also accept Medicaid. Emergency appointments available for those in pain; call for an opening.

Ottawa County Health Department - 12251 James St, Holland (616) 393-5694. MILES of SMILES mobile dental van provides free general dental services to eligible Ottawa County children; MIChild and Healthy Kids Medicaid dental provider.

Redi-Dental Emergency Dental Care - 12662 Riley St, Holland (616) 796-3200. Urgent dental services at competitive fees; accept Medicaid. Walk-in or by appointment.

- 1106 Fulton St, Grand Haven (616) 846-2701. Limited referral network for dental care; please call for information.
Grand Valley Dental Care - 2064 Baldwin, Jenison (616) 457- 2299. Medicaid, MiChild, Healthy Kids provider; general dental services.

Grand Rapids Community College - 143 Bostwick Ave. NE, Grand Rapids (616) 234-4237. Cleaning, fluoride, sealants, x-rays, periodontal therapy; $25 adult/$20 child. Call for appointment (no summer appointments).

Cherry Street Health Center - 550 Cherry St, Grand Rapids (616)235-7289. Emergency dental services here and at four additional locations in Grand Rapids (West Side, Ferguson, Belknap, Baxter). $20 for uninsured; also accept Medicaid. Services offered on a first-come, first-served basis beginning at 7 a.m. (days vary depending on location). Visit the Health Department website for additional locations and phone numbers.

Muskegon Family Care Dental Clinic
- 1700 Oak Ave, Muskegon (231) 767-9830. Uninsured fees based on ability to pay; Medicaid provider. Accepting children only as new patients; limited adult emergency care (with visible facial swelling) available by calling the clinic at 4 p.m. T, Th, or F.

Hackley Community Care Dental - 2700 Baker St, 3rd floor, Muskegon Heights. (231) 737-8603. Only accepts Medicaid. Emergency dental walk-in with pain/swelling. Bring proof of Medicaid and ID.


Substance Abuse Resources

Alcoholic’s Anonymous (AA) - Call (616) 913-9216 or visit www.grandrapidsaa.org for times and locations. Free support available for anyone with alcohol related problems, including friends or family members of persons with alcohol problems.

O.A.R. INC. (Ottagan Addictions Recovery) - 483 Century Lane, Holland (616) 396-5284. Provides outpatient counseling, assessment and therapy for substance abuser’s and their families.

Prescription Resources Mental Health Resources

Meijer, Target, Walmart, K-Mart, Sam’s Club, D&W, Family Fare – most large chain pharmacies offer free or low cost medications which vary by location. Call or visit their websites for an updated list.

Holland Free Health Clinic – 99 W. 26th St, Holland (616)392-3610. Assistance in procuring free prescriptions for uninsured who meet eligibility requirements. Holland/Zeeland area residents; by appointment only.

InterCare Clinic – 285 James St, Holland (616) 399-0200. Low cost prescriptions available to InterCare patients.

Love, INC Tri-Cities - 1106 Fulton, Grand Haven (616) 846-2701. Free, one time urgent need prescription; call or walk-in. Assistance in procuring free prescriptions for eligible uninsured under age 60 in NW Ottawa County.

North Ottawa County Council on Aging - Two locations: Grand Haven - 422 Fulton Ave (616) 842-9210 and Coopersville - 198 East St (616) 997-1786. Urgent need medications and free, year-long prescription assistance for eligible seniors age 60+ in northern Ottawa County. Also assist with Medicaid and Medicare enrollment.

NeedyMeds.com - Online source for prescription assistance to obtain low cost or free medications; must have doctor’s prescription. Visit website to see if your prescription qualifies.

Ottawa County Community Mental Health - Contact the Access Center to determine eligibility: (616) 393-5681 or (877) 588-4357. Mental health services for Ottawa County adults and children with Medicaid or without insurance; full services for those with severe/persistent mental illness or developmental disability; three locations.

Vision/Hearing

Goodwill Optical - 393 E. Lakewood Blvd, Holland (616) 393-0383. Eye exams for $32; new eye glasses with frames start at $68. Cash, Medicaid, Medicare. Call for an appointment Tuesday through Saturday.

Hospital Bills Health Coverage

Charity Care - for unpaid hospital bills that are not in a collection agency. Ask the hospital for information on this program. Client will need to have a Medicaid denial from DHS in order to apply for this assistance.

Ottawa County Health Department - 12251 James St., Holland (616) 393-5758. Medicaid enrollment assistance for children or pregnant women (MIChild, Health Kids, MOMS); Plan First enrollment for women under 44 needing family planning services.
