Environmental Emergencies In-Home Assignment

This portion of training is completed by a designated Environmental Emergency Trainer at your work site. Talk to your employer to arrange a time for this. Your employer will send the completed materials to the Training Center. Completion of this requirement will be noted on your training record.

Training Certificate Checklist

This checklist is completed by you and/or your supervisor. Schedule training with your supervisor, register and complete the class. After you have passed a class, indicate the date on the form.

Testing Out Procedure

If you have received training or education elsewhere which you believe meets the requirements for any of the courses, you may be able to test out of a class. Send a copy of your transcript, certification, or licensure to Carol Brown or Lorrie Hackney at The Training Center. Your supervisor may send you to the Training Center on the scheduled "Testing Day" (noted on the calendar). No "testing outs" will be done on the day of the class.

No Show, Cancellation and Reschedule Policy

A "No Show" is defined as a person who is registered for a class, but does not attend that class and the CMH Training Center is not notified within one full working day before the scheduled class or they arrive late and are not admitted to class. As soon as a supervisor is made aware of a cancellation, the supervisor should complete a "Class Registration/Cancellation" form and fax the form to the Training Center at (616)494-5541.

A "NO SHOW" FOR ANY CLASS WILL BE CHARGED $20.00 PER MISSED CLASS.

Registrations for future classroom trainings will be suspended if the $20 fee is not paid within a reasonable time period (60 days). Registrations will again be accepted after the fees have been fully paid.

Classes begin promptly at the scheduled time. Late arrivals will not be admitted into the class. They will be instructed to contact their supervisor to reschedule.

Class Cancellation Policy

If Holland Public Schools, West Ottawa Public Schools, or Grand Haven Public Schools are closed, training classes for Holland and/or Grand Haven are canceled. In addition, classes held at the Muskegon County facilities will be cancelled if unexpected facility problems occur during class time; for example, a power outage.

If there are less than 5 people registered for a class, it will be cancelled the day prior to the scheduled class. If you are registered and we have record of a current phone number, you or your supervisor will be called directly.

Training Expectations

  1. Trainees must sign the attendance sheet when they arrive for classes.
  2. It is expected staff will not work third shift prior to a morning/all day training.
  3. Visibly ill staff will be asked to leave the class to reduce the spread of infection.
  4. Trainees are expected to conduct themselves in a positive manner. This includes punctuality, attentiveness, and participation.
  5. Trainees must demonstrate competency throughout all skill demonstrations and must attain at least 80% on all written tests.
  6. Trainees who do not pass the written test or skill demonstrations may contact Lorrie at 494-5410 to schedule a retake test.
  7. Injury free training is the goal! If an employee has a pre-existing condition that would prevent him or her from safely participating in physical skills practice, for example, lifting and transferring techniques, CPR demonstrations on the floor or restraining techniques, the supervisor and trainee should evaluate whether the trainee can safely participate in physical skills practice. If it is determined that the employee cannot, the supervisor should contact the Training Center before sending the employee to training with a request to have the employee observe the physical skills practice. This action will be noted on the attendance sheet and employee’s training record.
  8. Cell phones and pagers can be used during break times in appropriate locations at each training site.
  9. Trainees should wear clothes that are appropriate for physical skill demonstrations. Skirts, dresses, high heels, and/or sandals are not recommended. Dressing in layers allows trainees to adjust for their personal comfort in the classroom.
  10. If there is an emergency, trainees can be reached by calling (616) 494-5410 or (616) 842-5350 (Grand Haven site).

For any training questions contact Carol Brown (494-5413) or Lorrie Hackney (494-5410) at the Training Center.
