A community health improvement plan (or CHIP) is a long-term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process.
Using the results of the recently completed Community Health Needs Assessment (CHNA) for Ottawa County, we will begin the process of developing the 2018 CHIP. This process will include the prioritization of our greatest public health issues, establishing desired outcomes for our community and ultimately recommending strategies for implementation.
We encourage participation from those working in the field of healthcare and public health but also strongly encourage other representatives from business, government, nonprofit, schools, and the faith communities to get engaged.
Three planning meetings will be held at the Main Conference Room at the County of Ottawa Administration building on the following dates:
* Thursday, April 19th (8:30 - 10:30am)
* Thursday, May 31st (8:30 - 10:30am)
